Where patients had blood transfusions before 1996, the inquiry have recommended they should be tested for Hepatitis C.

These transfusions would have taken place in setting other than general practice, therefore it is unlikely this will be in your GP records.

If you did have a blood transfusion before 1996, you can request a free home self-test, from https://hepctest.nhs.uk

You do not need to contact the surgery in this regard.
